I saw one flying around in Cyrodiil months ago, the biggest issue is that it is hard to know the name to report if they are that hard to target. On PC I think I figured out a workaround with addons, but unsure if it would be possible on console.
On PC-NA there was a whole group of them, and I think there was even a guild named barcodes or something like that. I doubt they are all cheating like the one I saw though.

A video or SS with the platform/server, location, and time should be sufficient for Zenimax to figure things out. Everyone can take an SS, and I'm pretty sure every platform can take a video.
It is also fairly common in some PvP games to have odd names or characters. Much of it cannot be prevented, as the examples I provided earlier.

Possible, but I doubt that alone would be able to identify them. Perhaps. I also only ran into this person once (I don't PVP much). I have heard mention of people flying around with scrolls and dropping them in the slaughterfish though. Since I don't PVP I don't really care THAT much, but it would be best to fix whatever they are doing to manipulate the game to fly around and teleport underground. I have seen videos of people doing this in trials too, but unsure if that was fixed.

I believe the complaint this thread is making is that a player is deliberately making their username illegible to avoid being reported.
