Extremely disruptive graphics glitches/distortions

I have been running into places in the game where it looks like the world is being torn apart or it's completely distorted. It's happened three times during trials. Like someone else said on the party chat once, I feel like I'm tripping balls.

You should run through the troubleshooting steps found here which can sometimes improve graphical glitches. If you don't see any improvement please let us know.
