I would imagine the most efficient way to "farm" worms would be grinding one of the popular zombie hotspots: one is in the NE corner of Greenshade east of the Verrant Morass wayshrine, and another common one is along the coast north of Sentinel in Alik'r Desert.

These help you out three-fold by gaining decent xp, leveling Fighter's Guild (I think), and the possibility of worms.

Murkmire has a glorious amount of grub baskets, if you don’t mind stealing (there are free-for-all ones too).

Saltwater bait is worms and chub. You can farm worms by killing the dead (undead) things such as zombies, skellies, draugr, etc. When you fish foul water, you will sometimes get chub.

Oh, and alchemy plants also have worms sometimes.

You can buy worms from guild traders (search 'bait') to get started and then farm more.

I think cloth nodes also drop worms, sometimes crawlers though.
