Bug charge or gap closers create falling from sky

Ive had this happen 4 times now in cyrodil where i dragon leap, or shield invasion skill into an enemy, get tossed into a loading screen, come to falling from the sky with some yellow orb i fall through, hit the ground somewhere else on the map, and then run around until another loading screen, and im back to where i started, usually dead from fall damage or the enemy.
