Random setting changes

This has been happening over the past week. Little things but they keep happening.
I'm on a PC and use an xbox controller
But basically certain setting keep changing on me.
Weapon swap disappears. I cant swap out my weapons and must go into settings controls and reset it back default. Then I can swap.
I had to reset being able to see a specific guild chat. I was wondering why none of my posts were showing up.
I can't aim caltrops anymore. Before it took 2 button hits to activate, one to aim and the next to fire.
Setting up siege machines is all kinds of jacked up. Again, before it took 2 button mashes to set up, the first giving me the circle thingy to place and then again to set up. Now it flashes and disappears. The first button hold brings up the quick slot menu instead
And I had some loot disappear, nothing big but something I was really happy to finally get. It was the last thing I got before logging off, went in the next morning excited to to try it out and it was gone.
