Attack freezing with Maelstrom weapons

I have been having problems lately with my character just all of sudden glitching and not being able to attack. Randomly, for no reason, he will just stop attacking at all and his arms will twitch/wiggle like he wants to attack but nothing will happen. Only thing that seems to break it is bar swapping back and forth several times and then it will break the effect, otherwise the only way to fix it is to die, which this glitch causes me to do quite often, lol.
Curious factor is that this ONLY started happening after I got and began equipping a single Maelstrom weapon on my duel wield bar, never before. I've had Maelstrom bow on back bar for awhile and never had this happen and now that I have added a Maelstrom dagger and a VO dagger on the front bar it happens quite often. Counted in a recent Maelstrom run that it cost me 7 lives in a single run... For no good reason. If you can't attack or use abilities you can cause damage or heal yourself and you're a sitting duck.
Is this a known glitch? Does it go away when you equip a second Maelstrom weapon onto the DW bar or is this just some BS you have to deal with in order to use Maelstrom weapons?
