That would be a pretty major disappointment if that were indeed the case. I don't see why the players should be punished due to the lack of (though recently improved) stability in Wrothgar

@Pepper8Jack Unfortunately, your Flawless Run was ruined due to your game crashing. Nothing else to it than that really unfair and definitely hope you can run it without a death again to eventually get your title!
Alight bro, don't listen to what Ben has said - It's only partly accurate.
"We are investigating reports from players expressing technical issues on console that can make [flawless vet maelstrom runs] difficult to achieve without re-logging, and we are working toward a resolution in the next major update."
Ben has made it sound like you need to re-log to attain the Flawless title, when all you need to do is NOT pick up the quest.
If you pick up the quest and DC it counts as a checkpoint on the quest when you go back in and is almost certainly related to when they made changes to that to help players who couldn't complete the arena in one pass (IMO content should never be changed to accommodate bad players)
Anyway - Go in and DO NOT pick up the quest and you'll get the title - DC or not.
Alternatively when you're in the lobby and the quest giver and merchant are there, do a hard reset of your xbox - and then pull your power cable out for 15 seconds before turning it all back on and you shouldn't get the DC at all. (I think this might be what Ben meant by Re-Log but I've always done a full reboot because it doesn't always work otherwise)
I actually had the same issue a long time ago and got the same response - "we do not have the ability to manually add achievement or titles to an account." made me laugh - They can add items and alter everything else but they can't add a 1 to the DB under what I assume is a has_flawless_conq column - OK.
Anyway, you've done it once now so it should be a walk in the park from now on - Good luck though.
Do it again, this time don't grab the quest. You should get the achievement even if you lag out/disconnect.
