Check your Mundus Stone, it may not be there any more. I think it was the IC update that reset everyone's Mundus.

Update gear to highest level available, make sure it is at the very least purple (blue if you are very skilled, even then, make at least the chest piece purple), try to get gold weapons if you can. Make sure it is enchanted, use a food/drink buff (I favour drinks, even for burst builds, because sustain is handy), make sure you have at the barte minimum some potions on hand for your primary resource, and make sure your build is made with a goal in mind, and choose armour/jewelry/weapon sets, enchantments, mundus, etc. with those things in mind.
Example of a goal taken from one of my builds is High stamina regen/High weapon damage. Another example from another build of mine is High health + health regen (for my tank)

Sounds like you ran into someone using the medium Armour skill, I think one of of the morphs is called Shuffle. This will make the player look like a shadow and give him a 20% dodge chance. I am not sure what the morphs add, but this is the base effect.
I have pumped overload into a Nightblade with this on. Out of about 15 shots, only 2 hit. Guy was lucky. I always try to match ambush spam with Overload spam haha.
