Weapon Glitches

Every few days at some point during combat I cannot use my weapon. The weapon is equipped but no amount of button pushing - Skill attack, Light or Heavy attack - will make my character swing his sword. I'm able to move around but I'm unable to unequip or swap weapons.
I've tried bringing up the Menu and/or the Map and occasionally that will fix it, but usually it doesn't. Of course my character is killed, and when I respawn usually the problem is solved, but occasionally it isn't. What's wrong?
