Voice Chat bug

Sometimes I can't connect to a chat lobby or it gets stuck in a join/exit loop. I can't hear anyone and no one can hear me.

Usually happens after changing the voice chat channel or switching between characters. I am using a headset and the XBOX One headset adapter.

    Been having this problem the last 2 or 3 days now. Keep having to relog. :/
