Hotbar graying out

I seem to be having this problem only on my Templar (NB seems fine so far). Every so often after I use Focused Charge (and sometimes even without using it), my hotbar goes gray (i.e. the skills on it turn dark) and the skills on it cannot be used. I have to keep hitting ESC to get to the menu screen and then hit ESC again to get out of it and then all my skills light up and work again.
This issue keeps getting me killed so often on my Templar that I think it might be a record.
Also Focused Charge keeps overshooting the target ALL THE TIME. Charge a target and then I need to turn around 180 degrees because I'm on the other side of the target from the Charge. 2H weapon charge and my nightblade teleport work just fine.
