Are they different accounts or if a forum account gets banned/warned then the players account will also get banned/warned? Who can give a definitive answer?
If you get a forum warning/ban that will apply negatively for your crown gifting request unless you have done the request long prior to the warning/ban. Talking from my own experience.
There's plenty of banned forum users still happily playing the game. Go wild!

I mean... a forum account is associated with a game account. Afaik at least, you *must* login to an actual ESO account on the support site in order to request forum access. So I personally wouldn't risk my ESO account by going nuts on the forums. But, no one can give a definitive answer except zos.

He is correct that they are associated, but they are still separate, which is what the original question asked. They do not use the same username or password unless we choose to set it to the same.
They're not going to lock you out of the game just for trolling or cursing but obviously if you go really far like try to start a witch hunt against specific players or members of the dev team they know who you are.
