Your connection speed means nothing. You could have bad SNR, poor QoS settings on your service provider end..
No, this happens to entire groups (in many different groups made of up many, many different players at different times of the day/night) while playing in the same instance on the same platform. On Xbox NA, it's a common occurrence, most often occurring in either Cyrodil or in trials. There is no way every single player has issues, but not the game.PC/Xbox NA
