Not sure if this is the right place to post, so please let me know if not.
During the Castle of the Worm finale (Encountering Tharn in person) I will randomly die. As soon as the speech encounter with Tharn is complete, mannimarco confronts him. I will instantly die and have to restart the encounter. The skeletons will not have spawned by this time btw.
Is anyone else having this problem/can they suggest a solution?

I have this same issue. I have tried 10 times at least. I first encounter it at level 20 and than level 33 and lastly level 44. As soon as he opens his mouth , boom I'm dead.
