Doubt this is Xbox specific. There is a bug where your character will get completely stuck in a heavy attack. This is serious combat issue that affects both PvE and PvP gameplay. Most frequently this happens to me on a magicka character with a staff. The only way to stop the attack is to block. Releasing right trigger to finish the attack doesn't work - the character will continue completing a heavy attack. Casting another ability to cancel the animation doesn't work - the character will continue completing the heavy attack. The only way to end the cast is to block.
