Whilst upgrading a white piece of medium armour to green using hemming using enough pieces to give 100% chance of success I suffered a failure and lost the piece and the hemming...how can something with 100% chance of success fail? Anyone had a similar thing or am I missing something here (does 100% not actually mean 100%)?

✭✭✭✭✭Are you sure you lost all of the tempers? I once set something to 100% chance of upgrade (5 tempers), and I saw it drop down to 20% chance (1 temper) as I was clicking to do the upgrade. The upgrade failed, but only 1 temper was used up. I've heard a couple of other people report that the same bug occurred for them.

✭✭✭✭✭Sometimes, the number of upgrading items you are using drops just before you confirm the upgrade. Maybe that happened to you and you didn't notice. A really annoying bug.Just so that everyone knows, my Altmer still can't have black hair. About a dozen of Altmer NPCs in the game have black hair. Just saying.

Sings-Many-Songs, Argonian fisher: Sure, I'll do the thing... Eventually. Maybe.
Aerindel, Altmer stormcaller: After extensive research, I've come to the conclusion that doing the thing would be a waste of resources.
Liliel, Dunmer pyromancer: Aerindel said I shouldn't do the thing. Something about "resources".
Gyda Snowcaller, Nord cryomancer: I will find a way to do it that won't waste resources and make Aerindel proud of me.
Beatrice Leoriane, Breton vampire: I persuaded someone else into doing the thing. You are welcome, dear.
Sahima, Redguard performer: Doing the thing sounds awfully unpleasant and really not my problem.

It's a bug, it happened to me too, I had to exit the black smithing window and reenter it to "fix" it. for some reason it resets the temper to 1 unit used and the popup window usually shows the % that it will temper at, i dropped form 100% to 30%.0

✭I've never failed at 100%, but I did fail 4 out of 5 tries with a 90% success rate one day...i kept thinking "this can't be possible, surely won't happen again" and would try again at 90% and fail....lol0
