Die and disconnect, didn't fixed for over one year

In group dungeon and trial, if I'm dead my character disappears and disconnected.
In addition, I got kicked out from the instance sometimes , can't loot from the boss, or failed to get the achievement.
I suffered this for over one year, ZOS are you know this problem?
Or should I don't die? LMAO
