Companion equipment disappear from companion UI menu. Can't access anymore

It doesn't mean that companion actually lost equipment. Just disappear equipment icon from UI menu.
Is it my local problem or common bug?

I have [1Tough tank] [1StamSorc-DD] [1Necro-DD] [1Real Healer]
with [1Stam Blade].
But, I'm Tank main player. Recently I'm doing Healer.

✭✭I haven't had invisible icons, but a day or so ago I did have trouble changing equipped gear. It wouldn't let me interact with the equipped gear the best I could do was double click an item from my inventory (right side) to swap what was equipped.
A /reloadui fixed it.
I play with addons so it's possible something went weird, but I hadn't experienced it before. And none of my addons that I'm aware of interact with Companion stuff.PC NA1
