'frozen' after ultimate & leap as Werewolf (return of old bug? )

Just now , while fighting the 'Sulpund Grange' WB on Vvardenfell map , i had my companion (Bastion) out with me and a few seconds after i morphed (ultimate'd) into my WEREWOLF form , my Bastion companion DIED from the WB's aoe attack....and then immediately i became totally frozen on-screen in 'leap' animation (with all my skillbar buttons blacked out) .

Also: i haven't tried to reproduce those exact circumstances i mentioned above ^^ yet, so it's possible my Bastion companion dying aspect was just a coincidence.
However, amazingly, this is the 1st & only time this 'freeze mid animation' bug has ever happened to me and i been here since 2013 BETA , so i figured it couldn't hurt to report it.
