[BUG] Vampire passive Undeath

I've been wondering when, if ever, does this passive proc. So I set up a bufftimer for it with visual and sound alerts, as well as recording a 20mins fight. It seems to be intermittent at best. In testing vs open world npc, just letting them hit me until I'm almost dead, heal up and let them keep hitting, it never seems to proc at all..
What's your experiences? Could we have a look at this passive please?
