Anyone else having issues with ultimates firing?

Since Blackwood me and several of my friends have been encountering issues were instead of LB and RB firing our ultimates our LB skill goes off instead.
Could be a compete coincidence. Maybe in the heat of battle we're all not hitting both at the same time, though this is a new occurrence.
