Set bonuses dropping off after death, after zoning, after combat etc

This bug keeps happening without fail, I don't even know if half the time my set I am wearing is working.
This has tested and shown to effect:
Heem-Jas
Heartland Conqueror
Deadly Strike
Prisoner's Rags
Coward's Gear
And I am sure many other sets with a special effect associated with the 5th set bonus. I cannot even for sure that it is not working with straight stat set gear.
This seems like a bug leftover from Cyrodiil proc disabling,. but has carried over even to anything PVE related. This is really making me not want to play.
