No credit for Veteran Blessed Crucible

This is a really weird bug. Today, Blessed Crucible is a pledge. I got a group together with a character that has never done it in vet, and we did it in vet. Started at the beginning, went through to the end. I had no disconnects, died once on the final boss.
I got the achievement for Hard Mode. I didn't get the achievement for Veteran. If I look at the achievements, it says I failed to kill the Troll King. Um. You cannot progress without killing the Troll King, and I was there all the time. So, Recna Mor Cena has veteran hard mode without having achieved veteran conqueror. How did that happen???

Can confirm this on PC/NA.

Just did a vet HM Speedrun with a character that had previously not ran the dungeon on vet.
~I did not get credit for "Grunt the Clever" or "Troll King" in the achievements screen, and as stated in the report above I too was there for the entire run.
~The Hard Mode and Speedrun are both being tracked as cleared, however from what I can tell the dungeon progress itself is for some reason not being recorded properly.

EDIT:
Took another character through, vet HM, not a Speedrun. This time the encounters all registered as properly completed and the achievement unlocked

Were you dead at the time the monster died? I wonder if that has anything to do with it... it shouldn't, but maybe it did?

No, I was alive right through to the final boss. Died once there. The troll fight actually went very smoothly. Maybe that was the problem. The game didn't believe it.
