Really weird light/heavy attack glitch

I've had this problem for a while now, but never really brought it up. After taking a hiatus from the game and coming back, it appears it's still there. Whenever I'm mid combat, occasionally my HA/LAs will break. By that I mean I can't just hold down the button for example. I need to release the mouse button, wait a second and then press it again. It almost means I can't quickly chain a heavy or light attack immediately after an ability. I need to wait a bit of time after the ability has played out or my character will just stand there and not register the mouse click at all.
It seems very random, so I'm not entirely sure what's causing it, but as you can imagine, it's pretty game breaking when it does happen.
Anyone else had a similar issue and know how to fix it, or if there's a fix?
