Sneak kill does not work anymore

A few weeks ago out of nowhere I could not sneak kill anyone. I play with a xbox one wired mini controller and when I push the Y and B button in stead of killing with the blade of woe it does one off the attack that are bind on those keys. I bought a new controller and it still does it. I tried doing it on the keyboard and still same thing. I repaired my game not long ago just to try it and it stil no change. It starts to get frustrating because I can't do any brotherhood quests now.

Is someone else having this problem? And have you found a fix?

    why is this getting zero attention from developers?
    best in slot: is the armor, weapon, clothing look, pet, or no pet, jewelry, race, class, skill choices, amount of DPS, and skin color best fit to you and the way you wish to play and makes you feel good and performs what YOU think it should be.

    worst in slot: what you read in zone chat, and forum comment, and forum thread, and you tube video, and live streamer advice, and class rep advice, and guild chat advice, and whisper told you to wear, and use for skill, and dress like, and use for weapon and armor.

    I have had a lot of instances where Blade of Woe wouldn't work. I think a lot has to do with the in-game LOS checking.

    -If you are approaching from an angle and they have some or all of their back to something
    -If you are approaching from a higher or lower elevation the LOS checking might be off
    -If they are moving you typically need to be closer so that the time delay between pressing the button and the game checking for a valid kill doesn't allow them to move out of range
    -If there is any sort of terrain feature in between, even if you can move through it, such as an archway
