Apparently there’s a issue at least on console where sometimes your voice chat leaves and joins endlessly causing the prompt to show up as well as the joining/leaving sound to play.
As I just dealt with this I thought I’d share the fix which is to go into text chat and type /reloadui while it can still come back I thought this may help someone.
