Tools of Slaughter quest bugged [PS4/NA]

The Tools of Slaughter daily quest in Murmkire is currently bugged on PS4 NA. Sometimes the third artifact (chalice), near the boss, will not appear, and I'm not able to complete the quest. Picking up the third artifact before picking up the second artifact seems to prevent the problem. According to this thread (https://forums.elderscrollsonline.com/en/discussion/441956/daily-quest-tools-of-slaughter-murkmire), the problem was supposedly fixed in Nov. 2018, but it happened to me a few days ago.
