[PC-NA] Choixth visually bugged in Murkmire

I finally ran into Choixth in Murkmire and I was so excited about finally getting this achievement done! I found her at the Black Rose Prison wayshrine and I read that you can just travel across the map and return to the BRP wayshrine and she would still be there to complete dialogue achievements.
Every time I came back, another Choixth would appear inside the previous Choixth I had already spoken to so by the time I got to the 5th dialogue, there were 5 total Choixths inside of each other! If you watch videos on this achievement, there is only one Choixth that shows up but I was seeing 5 by the end.
When I met Choixth for my 2nd dialogue:

This one is after I came back for my 5th and final dialogue. There were 5 Choixth's inside of each other but I couldn't get a better screenshot as she constantly moves around. This was the best one I ended up getting before she vanished. If you look closely, there are 4 heads. When she was moving around, there were times that you could see there were 5 but it only last a split second.
