No i have never accidentally dodge rolled backwards when i was at the top of a tower and was trying to avoid this night blade who was trying to kill me and ended up rolling completely off the tower and died... nope not me got me confused with another Templar

I don't think I have in ESO, but it's definitely happened in GW2. In the days before gliders and mounts, it could really suck to suddenly plunge off an edge.

In ESO I'm not as fast to hit the dodge key since I can't mouse-move to run forward so my left hand is pretty busy just moving me around the fight and hitting the skill keys. Also I seldom find myself fighting near edges that would make it a concern.
