Weird quickslot glitch

I am having a strange glitch. I open the quickslots and it has about 6 items there. If I am in combat and quickly press Q (on PC), the potion often doesn't fire. If I clear out the quickslots and have just the one I want, it works. Then I log out of the game, log back in later, and it has put all the items back in the quickslots. If I don't remember to remove them, I can have the same problem with potions not firing. Does it reload the quickslots for anyone else? And yes, I made sure the potion I wanted was the active item.
