These were added with dark brother hood.
Helmet means your gear needs repairing
The weapon means you need to charge your weapon (both if it stays when you weapon swap)
I believe the weapon icon (sword and bow) will also show up if you are using a weapon without any enchantment at all, if you don't have any poison equipped.
Basically, the game wants you to either have a non-depleted weapon enchantment or poisons equipped. If you have neither, the icon appears.

Not that I recall seeing. I've got a few new characters with the new slots open, all of them are using unenchanted weapons at the moment. No on screen warning.

This is correct. When levelling, most of my characters weapons i'd never enchant or use poison and it never came up. It's purely for running out of charge.
I guess I misinterpreted. Apparently all it's saying is that if you area using an enchanted weapon that's out of charges, it will NOT show the icon if you have poisons equipped instead.
- Added indicators for Armor Durability, and Weapon Charge/Poison.
- When any piece of equipped armor is broken, an indicator will appear near the ability bar.
- When a weapon for your current weapon set has no poisons equipped and runs out of weapon charge, an indicator will appear near the ability bar.
