Ignoring most of the time bugs, even sometime game is crashing while doing dungeons, pvp etc.

Seen it before (PC), actually I think it was after IC or Thieves update kinda regular that people fell trough floors into that blue void you just saw.
We would experience this very often:
Login on character at bank (or other location), immediately fall through floor (again and again for like a minute (you would see the people in the bank while passing again)), finally hit the floor dead (but at wayshrine). To avoid the sudden death when finally hitting the floor, blocking helped sometimes.
