Yesterday some of my guild mates and me were running Vet City of Ash. We finished Valkyn Skoria. Got the loot from the chest and a few seconds after we were all kicked off the server. All of us were back at the character selection screen. One of our DPS got the Valkyn Skoria helmet but it was gone. Two of us were still inside the dungeon with Valkyn Skoria still alive, the other two (me and the DPS who got the helmet) were outside. We reentered the dungeon and it was reset back to zero. Every single mob and boss we killed were back while the other two of us were in front of the final boss.
