I've lost count how many times, on accident I've attacked a guard...

Why no distanced names above the person or animal or whatever? I've come around a rock and accident started to attack guards I thought were enemies. I logged off last night because I didn't feel like waiting and getting attacked by other guards. Implement something... Games been out for a few weeks and I haven't seen 1 update?? WTH is going on?

The solution is simple. Go to the settings and turn off "Attack Innocents".

Theres actually an option in the game settings to prevent attacking friendly targets!
There will apparently be an update around the end of July - or so its being said. :c.

I know on PC there's an option in the settings to disable attacking innocents. I would assume you have that on consoles as well? I would suggest using it. Also, don't you have a target icon when you're attacking something? Also, guards will never flash red unless you're fighting one.

It might be good to check who you are attacking before attacking them. It is still possible to accidentally hit an innocent but a bit of care can help.

Option was in settings the entire time, including an option to turn on health bars and differentiate between NPC/players based on health bar colour. To put it right back to you from the devs perspective:

"Why can't players look into settings before complaining? Game has been out for a few weeks and they honestly never thought to look through the options? WTH is going on?"

Because it's automatically built that way in all the other MMORPG's I've ever played. No one is "complaining", it's called a suggestion, for which I did not know there was already an answer to.
Thanks for the awesome input though!

I have that flaming chain on my Dragon Knight and have accidentally hit X and pulled an innocent to me. I actually killed a farmer last night, that attack latches on to anything in its vicinity.

It's real simple, people. Turn off the option to attack innocents in the settings. It should be off by default, but it isn't.

Also, is there a way to make it so when I'm attacking with my sword, it sticks on one target instead of skipping around and giving everyone a hit one at a time? This has killed me a bunch of times as well. I start hitting people one by one, instead of being able to focus just on one enemy. The fighting in this game is horrible if you ask me.

No, there is no way to "lock on" to a particular enemy. You can target them with the tab key on PC, but it doesn't lock onto them.
