Failed: logout ...?

'Failed: logout' message every time my main logs out no matter where she is, and my personal favourite 'Failed: Death' ...? Well, of course I did actually die, lol.
I have also noticed that upon login or zoning my main appears to kneel down and moves as though she is rifling through something. Any ideas what this could be? It's only happening on the one (of my eight) characters.

Do you happen to be running any add ons for the game? With the recent introduction of 1.5.8, it's possible that one the of add ons has become unstable with the new code. Please try running the game without any add ons and let us know if the same issue occurs.1 -
