Game Crash when I enter in new area or Fast travel.

I am having game crash issue from past few days:
1. If I choose travel to a group player the game crashed to launcher. Even sometimes it happened via wayshrine travel.
2. and whenever I enter in new area.
Now a days I am getting a decent FPS and everything looks normal but still this game crash make me mad.
