Random Health Increase Pool?

So I was playing a while ago and found a random "pool" almost, just laying around outside by the water near some ruins, and when I investigated it, it random leveled up my health bar quite a bit, like 1/4 increase. I have no idea where it was and I can't replicate it with my other characters- What was it I found? Help?
