Bookworms are malnourished!
Have you seen those bookworms in Mages Guild? They're nourished with knowledge and the rations are always stolen by Sheo's banekins! Lock yourself up in the dragon break chamber and read all you want! Just make sure you replenish the supplies of soul gems used to power the chamber.
Allows tweaking in Slim direction only.

The Ale keeps you warm!

Meridia's secret
Somewhere in Cold Harbour lies Meridia's closet, opening which would allow one to explore the secret of Meridia's beauty. However, everyone knows her deal, a body for a body. Who will you be sacrificing? With that being said, these daedric rituals might have adverse effects, you've been warned. *Rumors say the closet would work better with a dead necromancer as offering.
Allows tweaking of body part sizes.

People keep telling me they're gonna buy me an ale. They never do.

There are only two things I can't stand in this world. People who are intolerant of other people's culture. And the Elves.

Questing in the game should be enough to burn off fat and prevent you getting too twig like (muscles), as for the Ale thing, um No.

Sacrificing someone to stay beautiful would fit in with the Lore, but not with Meridia, Namira or Boethia would fit better.
People believe what they either want to be true or what they are afraid is true!

Haha, thought of both your concerns.

Anyways, I do feel that the idea of implementing food contributing to weight would be extreme and should one log in after a week, they'll have a fat character.

Namira/Boethia would gladly accept the sacrifice, but Meridia is the beauty, perhaps it should be bringing her the corpse of worm cultists?

Ok, if it's an extreme thing, it could work, if they are logging in, eating a food for the boost and maybe only doing a few dungeons, then the weight gain would work.

Meridia maybe the beauty as you say, but she is also the most benign of the Deadra, which is why I suggested Namira and Boethia. Especially Boethia as he/she would delight in the deception. But the worm cultist might do too, they have plenty of weird lore to draw upon.

(I'm being ambiguous about Boethia's gender since books in Skyrim and Oblivion refer to Boethia as both)

@AlexDougherty‌ Maybe Boethia would work for a gender change feature. LOL.

Despite how extreme to base weight on player activity, it would be really interesting if they could implement it. I could never forget San Andreas.
