"Failed" Dark Sacrament After 100% Completion - Sewer Tenement

I have previously reported this as a bug in game multiple times. After destroying the Hookah FIRST, then killing the target with the Blade of Woe SECOND, then leaving the zone before the Overseer Arrives WITHOUT alerting the residents (0/5), the message FAILED appears before leaving the zone. I then received a GREEN coffer as a reward.

Totes-Bode✭✭✭This happened again in Smuggler's Den. After completing the objectives and leaving before the Overseer Arrives (there were no further objectives), upon reaching the door to the zone the message "FAILED: Complete optional objectives" appears. Reward: Green coffer.0
