PS4 / PS4 Pro North American server multiple disconnects

Since the PS4 Pro launch I have experienced multiple and I'm talking over 50 blue screen disconnects;
I have had this happen on the PS4 PRO and on my PS4;
I have experienced these at;
Crafting station in multiple zones including Craglorn, Stormhaven, Reapers March
Using the ingame UI to access inventory
Interacting with a wayshrine
Standing in the middle of a city doing nothing besides talking to guild mates
In several dungeons including Crypt of Hearts, Banished Cells, and Eden Hallow 1 and II
I have sent video or a bug report almost every time my PSN is the same as my account name here if that helps anyone locate any data that might help you find the problem.
