PS4 broken Activity Finder

Have had multiple issues recently with the Activity Finder.
1. If someone drops out prior to porting to the dungeon it's not allowing to queue for a replacement. Not sure if this is every time or not (since I'm not usually the group leader).
2. It's not always showing a notification about even being in a group, either on the lower centre of the screen or the top right (like a mail notification) - so if you're lucky you hear the noise, but if not you suddenly get ported to the dungeon when you didn't even know it had found a group. This could be if you're in a menu at the time (again, not sure).
