Lag and combat issues

Whenever I'm in a 'high level' area (30+) my attacks and/or abilities don't hit the opponent around 50% of the time, in some extremities I will cast Burning Embers 6-8 times and none of them will hit, why is this happening and is there a way to fix it/ is it getting fixed soon? This is totally ruining the later stages of the game for me as I have explored all of the lower level areas now, and combat is almost unplayable in the higher level areas, I often find myself needing to be in a group to not die when fighting 3+ NPC's at once..
