I also got a reply from Madison. It's now been almost 7 hours since the ticket was marked resolved, and I'm still getting the suspended from site message when trying to log into my account. -

The issue you are having with submitting a support ticket through the web assistant could be browser related. Please give the following a try and see if any of these allow you to proceed. -

Nope. I click on account (via forum page) , then I am redirected to the account site where first I have enter my birth date when I click submit, I get
